(1) I give permission for my child to participate in the Awana program at New Beginnings Church. I understand this activity will be closely supervised. However, should there be an emergency situation; I give my written consent to all supervisors to make decisions concerning the medical treatment of my child. I understand I will be notified immediately in the case of an emergency, and I agree to not hold New Beginnings Church, it's Awana leaders, employees and volunteer staff liable for accidents or injuries to my child.
(2) I give New Beginnings Church permission to publish in print, electronic, online, or video format the likeness or image of my child.
(3) I give permission for Awana Leaders and Staff to contact my child by written communications or phone calls to discuss club activities.

All above authorizations shall remain effective until June 15, 2020 unless sooner revoked in writing and submitted to the Commander.
